These forms are to be completed only AFTER you have spoken with our intake staff, and have been provided with an initial appointment. DO NOT COMPLETE THESE FORMS BEFORE SPEAKING WITH AN INTAKE STAFF PERSON.

This paperwork MUST BE COMPLETED PRIOR TO YOUR FIRST SESSION. If it is not completed and submitted prior to the first session, whether online or in person, your appointment will be rescheduled.

The forms may be password protected. You should receive a password from your intake staff person so that you can access these forms.
